The position includes a pathway to partnership after 1 year of employment for experienced physicians (2 years of employment for newly licenses physicians). Clinical Positions
The Care Manager is a member of a team of professionals dedicated to providing quality healthcare to a patient. The Care Manager is responsible for providing patient outreach and care coordination for patients with moderate to complex medical needs under minimal supervision. Successful candidate holds a current certificate as a Certified Medical Assistant (from the American Association of Medical Assistants), or as a Registered Medical Assistant (from the American Medical Technologists Institute of Education), or holds a current Pennsylvania Nursing License in good standing.
Care Manager Positions are available at the following locations:
- FT Clinical Position (LPN/RN) available in our Pennridge office. This position is 40 hours per week. Monday-Friday 9am-5pm or Monday-Friday 9:30am-5:30pm.
The Office Nurse is a member of a team of professionals dedicated to providing quality healthcare to a patient. The Office Nurse participates in patient care by assisting the physician, performing various nursing tasks, and acting as a communication link between the patient and the physician and other healthcare providers in the community. Successful candidate holds a current PA nursing license (RN or LPN) in good standing.
Office Nurse positions are available at the following locations:
- FT Clinical Position (LPN/RN/CMA) available in our Lower Salford Office. Must have a valid PA LPN, RN, or CMA license in good standing. Applicants must have background clearances.
- Position A hours are Monday from 8am-8pm, Tuesday and Friday from 8am-5pm, Wednesday from 8am-1pm, and rotating Saturday mornings from 8:30am-1pm.
- Position B hours are Monday, Wednesday, And Friday 8am-5pm, Tuesday 8am-1pm, and rotating Saturday mornings from 8:30am-1pm.
- Position C hours are Monday and Thursday 8am-8pm, Wednesday 8am-5pm, Friday 1pm-5pm, and rotating Saturday mornings 8:30am-1pm.
- FT Clinical Position (LPN/RN/CMA) available in our Pennridge Office. Must have a valid LPN, RN, or CMA license in good standing. Applicants must have background clearances. This position is full time-40 hours per week with an evening and a rotational Saturday morning.
- FT Clinical Position (CMA/LPN) available in our Upper Perkiomen Office. Must have valid PA CMA/LPN License in good standing. Applicants must have background clearances.
- Position A is 36.75 hours per week. Hours will be Monday 11am-8pm, Wednesday, Thursday, and Friday 7:45am-5pm, and rotational Saturdays from 8am-11:30am.
- Position B is 36.75 hours per week. Hours will be Monday 11am-8pm, Tuesday, Thursday, and Friday 7:45am-5pm, rotating Saturday mornings from 8am-10:30am.
- Position C is 36.75 hours per week. Hours will be Monday, Tuesday, and Friday 7:45am-5pm, Thursday 10:45am-8pm, and rotational Saturday mornings.
- FT Clinical Position (LPN/RN/CMA/RMA) available in our Western Bucks Office. Must have a valid PA LPN, RN, or CMA/RMA license in good standing. Applicants must have background clearances. The position is 40 hours per week with an evening and rotational Saturday morning.
- FT Clinical Position (LPN/RN/CMA) available in our North Willow Grove Office. Must have a valid PA LPN, RN, or CMA license in good standing. Applicants must have background clearances. The position is 37 hours per week. Hours will be Monday 12pm-9/9:30pm, Tuesday off, Wednesday 8am-7pm, Thursday 8am-6/6:30pm, Friday 8am-3/3:30pm, and rotating Saturday morning from 8:30am-12:30/1pm.
- FT Clinical Position (LPN-Assistant Clinical Supervisor) available in our Upper Perkiomen Office. Must have a valid PA LPN license in good standing. Applicants must have background clearances. The position is 37 hours per week. Hours will be Monday, Tuesday, Thursday, and Friday 7:45am-5pm, and rotational Saturday mornings 8-11:30am.

The Medical Assistant (Clinical) is a member of a team of professionals dedicated to providing quality healthcare to a patient. The Medical Assistant (Clinical) participates in patient care by assisting the physician, performing various nursing tasks, and acting as a communication link between the patient and the physician and other healthcare providers in the community. Successful candidate holds a current certificate as a Certified Medical Assistant (from the American Association of Medical Assistants or National Association of Health Professionals), or as a Registered Medical Assistant (from the American Medical Technologists Institute of Education) or the equivalent.

The Clinical Supervisor is a member of a team of professionals dedicated to providing quality healthcare to a patient. The Clinical Supervisor is responsible for duties such as: maintaining staff work schedules, evaluating personnel, training & orienting new employees, preparing & monitoring compliance with clinical department budget, recommending changes to work flows and processes, communicating effectively with the public, staff and providers. The Clinical Supervisor will also be responsible for reviewing quality metrics and ensuring proper steps are being taken, as well as nursing duties such as: performing EKG’s as needed, vaccines, updating patient charts, rooming patients as needed, etc.
Successful candidate will possess leadership skills and the ability to work well individually and as a team. Candidates should also be a go-getter and a self starter. Applicants must have background clearances. A minimum of 2 years experience in a busy family practice is required.

Administrative Positions
The Office Manager is and administrative team member who oversees both the admin and clinical teams within the office. The position works closely with the physician shareholders to implement, manage, and monitor various initiatives within the office. Responsibilities include (but are not limited to) scheduling team members, approving payroll and PTO, monitoring overtime, training team members, hiring, firing, and maximizing efficiencies within the office. A successful candidate has experience within primary care, has some knowledge of value based primary care, and excels in a fast-paced environment.

The Population Health Specialist works with our clinical and administrative teams to develop, implement and monitor workflows in the office to achieve desired outcomes as identified by the various payor value-based contracts. The population health specialist participates in office meetings, working with providers and team members to improve efficiencies and continue to educate teams in various initiatives.

Administrative Team Members have a variety of roles within our organization. Duties include (but are not limited to) checking patients in upon arrival, answering telephones to schedule appointments or record detailed messages for the clinical teams, checking patients out, managing in-office medical records requests, and always providing the patients with an excellent experience of care. Administrative team members may be tasked with billing or assisting with population health activities within the office. TriValley offers both Full Time and Part Time positions within our Administrative Teams.

- PT Administrative Position available in our Indian Valley Office.
- Position A is 22 hours per week. Hours are Monday-Thursday 8am-1:30pm and Saturdays 7:45am-12pm.
- Position B is 27 hours per week. Hours are Monday-Wednesday from 8am-5pm and every other Saturday 7:45am-12pm.
- FT Administrative/Billing available in our Western Bucks Office. Position is 40 hours per week. Hours are Monday-Friday 8am-4pm with a rotational Saturday morning.
- FT Administrative Position available in our Franconia Office. Position is 40 hours per week, and hours are to be determined. Candidate can expect a rotational Saturday morning shift.
Central Business Office Position:
- FT TCM Coordinator Position is available. This position will include: obtaining medical records post hospital discharge, monitor HSX and hospital portals for admissions, incoming faxes, and documents, communicating with care managers in each TriValley Office, and maintain a working relationship with area hospitals and their medical record departments. Position is 40 hours per week Monday-Friday 8am-4pm.
- FT Practice Transformation & Analytics Specialist. This position is responsible for monitoring compliance with and performance in value-based contracts within TriValley Primary Care. The position works closely with TriValley’s Practice Administrator, Clinical Committee, Population Health Specialists, Care Managers, and Supervisors, and is able to review and interpret reports from insurance companies to assess current performance and make recommendations for practice transformation efforts. The position requires a strong understanding and proficiency with numbers, percentile rankings, and general math concepts. Experience in population health and value-based contracting is beneficial. This position is on-site, and will require travel to outlying office locations – valid PA drivers license is required.
